Discipleship Commitment Statement

I, ___________________ have accepted Jesus Christ as my Lord and savior of my life. I believe in my heart and confess before others that Jesus was crucified for my sins and was raised again from the grave and is now living forevermore.

Today I am willing to make a further commitment to my heavenly father and to my Christian family.  I am committing to become his disciple.
